Overview

The Registered Nurse delivers safe, effective, and compassionate nursing care in accordance with the nursing scope of practice and individualized Plan of Care. This role involves direct clinical care, timely documentation, clear communication, and coordination with clients, families, and clinical leadership to ensure high-quality service delivery.

Primary Responsibilities

Field Case Management

  • Assess mental, physical, and emotional status, creating plans of treatment and coordinating care, collaborating with the Nurse Supervisor or Manager of Clinical Practice.

Documentation and Communication

  • Report changes in client medical status and emergencies promptly to the Nurse Supervisor or Clinical Manager.
  • Complete required clinical documentation accurately and on time using the electronic medical record system, ensuring compliance with regulatory and organizational standards.
  • Communicate schedule changes such as tardiness, vacation, requested time off, change of status, or reduction in hours to Care Coordinators promptly to support appropriate coverage.
  • Confirm weekly schedules with the Care Coordinator to ensure required staffing levels are maintained.
  • Perform additional duties as assigned.

Qualifications

  • Current, active RN license in the state of practice, in good standing.
  • 2 years of home health experience preferred.
  • Proficiency with OASIS documentation.
  • Strong communication and documentation skills.
  • Proven ability to work collaboratively as part of a healthcare team.
  • Commitment to patient centered, safe, and compassionate care.

Work Environment

  • Patient care is delivered in diverse settings, including private homes, assisted living facilities, and clinical environments.
  • Regular activities include standing, walking, bending, reaching, and assisting with patient transfers using proper body mechanics.
  • Daily travel to patient locations is required.
  • Routine exposure to blood, body fluids, and other potentially infectious materials. Universal precautions and PPE are required.

Physical Requirements

  • Safely lift, carry, push, or pull up to 35 pounds.
  • Assist with patient transfers and ambulation.
  • Tolerate prolonged periods of standing and walking.
  • Respond quickly and effectively in emergency situations.
  • Travel to patient locations throughout the workday.

OSHA Classification

Category 1, Regular exposure to blood, body fluids, and communicable diseases. Universal precautions and PPE are required.
