Matrix Medical Network

The organization serves health plan members across Exchange, Medicaid, and Medicare Advantage plans through a national network of nurse practitioners who conduct Comprehensive Health Assessments (CHAs) in patients' homes. Using an advanced technology platform, Matrix's clinicians visit members to obtain a deeper understanding of their health challenges and the support they need. For nursing professionals, Matrix offers a role centered on one-on-one patient care with significant autonomy. Nurse Practitioners perform adult and geriatric health assessments including history, diagnosis, treatment, education, point-of-care and lab-based testing, and care coordination with physicians and care teams. Visits are typically up to one hour and are self-scheduled, offering flexibility in days, evenings, or weekends. Compensation is structured on a per-visit basis at $95 per completed assessment, with a sign-on bonus of up to $1,500. Matrix actively welcomes new graduate NPs and PAs, making it one of the more accessible entry points for advanced practice clinicians seeking community-based care experience.
