Registered Nurse (RN) Field Case Manager

Continuity Care Home Nurses, Inc., an RN-owned private duty home care agency serving Southern California since 1991, is seeking experienced Registered Nurses to join our clinical team on a per diem basis. Responsibilities include conducting new patient assessments and admissions, completing comprehensive nursing evaluations, performing dressing changes, blood draws, and skilled nursing visits, managing Workers' Compensation cases and follow-up visits, coordinating patient care with physicians, insurance companies, case managers, and family members, supervising LVNs, CNAs, and CHHAs as appropriate, monitoring lab results and communicating changes in patient condition, completing recertification and follow-up visits, providing clinical reports to the Director of Patient Care Services, and maintaining accurate and timely clinical documentation.
