QA/Office RN

Continuity Care Home Nurses, Inc. is seeking an experienced Registered Nurse Case Manager to join our North Hollywood office team. This is a full-time office-based position with hours of 10:00 AM to 7:00 PM, Monday through Friday. Candidates must have prior private duty home care experience and be willing to perform occasional home visits or patient shifts during emergencies.

Responsibilities include managing and coordinating private duty patient cases, conducting patient admissions and assessments, coordinating care with physicians, case managers, insurance adjusters, and family members, scheduling and monitoring recertification visits, reviewing clinical documentation for accuracy and compliance, following up on laboratory results and physician orders, supervising field clinicians and caregivers, assisting with staffing and patient care coordination, providing clinical support to office and field staff, and participating in emergency patient coverage when necessary.
