RN Assessment Nurse

*Job Overview* We are seeking a dedicated and highly skilled RN Assessment Nurse to join our dynamic healthcare team. In this vital role, you will be responsible for conducting in-home comprehensive patient assessments to determine appropriate care plans, facilitate and coordinate with multidisciplinary teams. Your role will ensure patients receive the highest quality of personalized care. *Duties*

• Perform detailed patient Start of Care assessments and re-certification assessments including vital signs, fall risk, and home saftey assessments.
• Collaborate with case managers, and other healthcare professionals to develop and update individualized treatment plans.
• Utilize clinical knowledge in specialties such as geriatrics, behavioral health, and infectious disease care to provide expert assessments.
• Support utilization management efforts by reviewing cases for appropriateness of services and resource allocation.

*Skills*

• Excellent patient service skills with a focus on compassionate patient assessment and case management.
• Ability to interpret vital signs accurately and perform comprehensive patient evaluations across diverse clinical scenarios.
• Familiarity with medical records management, discharge planning processes.
