About the Role
As a Weekend Charge Nurse, you will be responsible for managing the care of our patients and residents on weekends, ensuring that their needs are met and their care plans are implemented effectively.
You will work closely with our interdisciplinary team to coordinate patient care, develop and implement individualized care plans, and provide guidance and support to our nursing staff.
You will also be responsible for conducting patient assessments, developing emergency response plans, and ensuring that our facility is in compliance with all relevant regulations and standards.
Responsibilities
Oversee the delivery of patient care on weekends, ensuring that all patients receive high-quality care and services.
Develop and implement individualized care plans, in collaboration with other members of the interdisciplinary team.
Conduct patient assessments, identify areas of concern, and develop emergency response plans as needed.
Provide guidance and support to our nursing staff, ensuring that they have the resources and training they need to provide excellent care.
Coordinate patient transfers and discharges, ensuring a smooth transition for our patients and their families.
Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
Collaborate with other departments, such as therapy and dietary, to ensure that patient needs are met and that care is coordinated.
Requirements
Current RN license in the state of Colorado
Minimum of 0 years of experience in a long-term care or rehabilitation setting
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals
Ability to work independently and as part of a team, with a focus on providing exceptional patient care
Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
