We are seeking a dependable part-time LPN to provide skilled nursing visits in patients' homes. $25.00 per visit (approximately 40-45 minutes) and $37.50 per visit on designated holidays
Flexible scheduling based on patient needs and nurse availability
Paid travel time between patients at $12.50 per hour, plus mileage reimbursement when using a personal vehicle (Arista nurse vehicles include an assigned gas card)

Responsibilities of the Licensed Practical Nurse (LPN)
Provide skilled nursing services under the direction of a registered nurse and in accordance with authorized orders and the established plan of care
Observe and document the patient's condition; implement ordered nursing interventions; and contribute to the planning, implementation, and evaluation of care
Administer medications and treatments and provide other ordered skilled care within the LPN's scope, training, and demonstrated competency
Provide or reinforce patient and family education and communicate promptly with the RN, physician, and other members of the care team
Complete accurate, timely documentation and follow agency emergency procedures
Qualifications for the Licensed Practical Nurse (LPN)
Graduate of a state-approved practical nursing program
Current, valid Ohio LPN license in good standing
Available for part-time, per-visit work, including rotating weekends with other LPNs
Current CPR certification; valid driver's license; current auto insurance; and reliable access to a vehicle
Successful completion of required pre-employment screening, including BCI background check, BMV check, Ohio Board of Nursing license verification, and drug screen
Basic computer skills and ability to complete electronic clinical documentation
