How you'll make an impact in this role

Deliver Compassionate Patient Care: Implement and actively monitor individualized care plans to keep patients safe, comfortable, and informed throughout their treatment journey.

Coordinate Multidisciplinary Care: Lead cross-functional collaboration across care teams to ensure seamless communication, prompt execution of medical orders, and holistic patient support.

Track and Communicate Progress: Monitor vital patient conditions and advocate for patient needs through clear, timely updates to families, physicians, and care team members.

Champion Seamless Transitions: Assess patient needs early to coordinate comprehensive discharge plans that ensure smooth, safe transitions from hospital to home or continuing care.

What minimum qualifications you'll need

Licensure / Certification / Registration

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education

Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

  • Registered Nurse credentialed from the Indiana Board of Nursing
  • RN License - Indiana Board of Nursing
  • BLS Certification - within 1 month of hire
  • ACLS Certification - preferred
  • Nursing Diploma from accredited school/college of nursing
