Your future role at a glance: Location: Evansville, IN Facility: Ascension St. Vincent Evansville Department/Specialty: Surgical Trauma ICU Schedule: Full Time | Days | 7am-7pm How you'll make an impact in this role: Deliver Compassionate Patient Care: Implement and actively monitor individualized care plans to keep patients safe, comfortable, and informed throughout their treatment journey. Coordinate Multidisciplinary Care: Lead cross-functional collaboration across care teams to ensure seamless communication, prompt execution of medical orders, and holistic patient support. Track and Communicate Progress: Monitor vital patient conditions and advocate for patient needs through clear, timely updates to families, physicians, and care team members. Champion Seamless Transitions: Assess patient needs early to coordinate comprehensive discharge plans that ensure smooth, safe transitions from hospital to home or continuing care.
