Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and detailed health histories to determine individual needs.

- Develop and implement personalized care plans aligned with medical directives and patient goals, ensuring compliance with healthcare standards such as HIPAA and NCQA.
- Administer medications, including injections and IV infusions, following aseptic techniques and safety protocols.
- Provide specialized care for patients with complex conditions such as dementia, Alzheimer's disease, or developmental disabilities, ensuring safety and comfort.
- Manage wound care procedures including suturing and ostomy care while maintaining sterile processing standards.
- Educate patients and families on disease management, medication administration, nutrition, and self-care techniques to foster independence.
- Utilize EMR/EHR systems like Epic or Cerner for accurate documentation of patient interactions, medical records management, and case management activities.
- Coordinate with multidisciplinary teams for discharge planning, long-term care arrangements, or hospice & palliative medicine services.
- Respond promptly to urgent triage calls or acute pain episodes while adhering to critical care response procedures.
- Ensure infection control practices are rigorously followed during all patient interactions to prevent cross-contamination.
Experience
- Valid RN licensure with current state registration required; experience working in home health settings strongly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Cerner; experience with medical documentation is essential.
- Prior clinical experience in diverse settings including hospital medicine, ICU or trauma centers (Level I or II trauma experience), emergency medicine, or critical care environments is highly desirable.
- Hands-on experience with IV insertion, medication administration (including botulinum toxin treatment), phlebotomy, and specimen collection.
- Background working with individuals with disabilities or in memory care units enhances your ability to deliver empathetic patient-centered care.
- Knowledge of infectious disease protocols, infection control practices, and aseptic techniques is required for safe practice.
- Familiarity with coding systems such as ICD-10/ICD-9 and CPT coding supports accurate documentation and billing processes.
- Experience managing complex cases involving chronic conditions like dialysis patients or those requiring ventilator support is advantageous.
- Strong understanding of nursing standards related to hospice & palliative medicine, trauma medicine, geriatrics, pediatrics (including neonatal resuscitation procedures), and post-acute care environments enriches your clinical versatility.
