About this role
Guiding Light Hospice is seeking an experienced RN Case Manager who brings clinical strength, consistency, and compassion to every patient interaction. The case manager plays a critical role in the patient and family experience. Provide skilled nursing care and case management for hospice patients. Develop and implement individualized plans of care. Coordinate with the interdisciplinary team to ensure continuity of care. Educate and support patients and families.
