  • Graduate of an accredited school of nursing.
  • Current, valid Registered Nurse (RN) license in good standing in the State of Hawaii.
  • Active RAC-CT certification through the American Association of Post-Acute Care Nursing (AAPACN).

Kahu Malama Nurses is seeking an MDS Coordinator to join a growing and dynamic team!

Job Description

  • Oversee the assessment and care planning process in the nursing department.
  • Ensuring that MDS (Minimum Data Set) is accurate, complete, follows guidelines (for Medicare and Medicaid), and is submitted to the appropriate agency or provider.
  • Coordinate resident admissions and initial services with members of the team.
  • Provide general nursing care to residents on an as-needed basis.

Position Requirements

  • Working knowledge of the Minimum Data Set (MDS and the Resident Assessment Instrument (RAI).
  • Proficiency with basic computer functions.
  • Proof of annual tuberculosis clearance in accordance with state regulations.
  • Valid CPR, First Aid, and AED certification.
  • Knowledge of Resident Assessment Instrument (RAI) guidelines and Medicare/Medicaid reimbursement processes.
  • Ability to solve problems under minimal supervision.
