About this role
We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care.
Responsibilities
Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.
Conduct regular performance evaluations and provide constructive feedback to staff.
Facilitate clinical training and professional development opportunities.
Collaborate with interdisciplinary teams to develop and implement patient care plans.
Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
Assist in resolving clinical issues and addressing staff concerns.
Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
Performs other duties and activities as delegated by the Clinical Director.
Qualifications
RN licensure in good standing.
At least one year of management or supervisory experience in the home health setting. Strong leadership and interpersonal skills.
Familiarity with OASIS, care documentation requirements, and interdisciplinary coordination.
Experience using electronic medical record (EMR) systems such as Kinnser.
Knowledge of clinical best practices and regulatory standards.
Commitment to maintaining patient confidentiality and ethical standards.
