About this role
This is an excellent part-time opportunity for an experienced Registered Nurse to join a growing team of outstanding geriatric care managers in the greater Chicagoland area. Care Managers must drive and be willing to travel throughout the Chicago area and western suburbs. This position pays $55.00 per hour. All client-related travel is reimbursed at $25.00 per hour.
Responsibilities
Conduct initial assessments of new clients to determine current medical, emotional, and social needs. Perform comprehensive in-home assessments for prospective Home Companion Solutions clients to determine appropriate levels of care and develop personalized care recommendations. Conduct annual supervisory visits for Home Companion Solutions clients to evaluate changes in condition, ensure care plans remain appropriate, and recommend adjustments to services as needed.
Develop a rapport with clients to become a trusted care provider. Provide critical care management and coordination for clients in various healthcare settings, including emergency rooms, behavioral health units, ambulatory clinics, physician offices, and intensive care units. Conduct comprehensive assessments of clients' medical conditions, mental health status, and social support systems.
