Great Paragon Health Care is seeking a dedicated and experienced Registered Nurse (RN) Field Nurse Supervisor to join our growing pediatric home health team.

This is a field-based supervisory position responsible for providing clinical oversight to our pediatric clients and ensuring high-quality, compliant care in the home setting.

  • Provide supervisory visits every 60 days for assigned pediatric clients.

  • Complete comprehensive OASIS assessments and required clinical documentation using the Axxess EMR platform .
  • Assess the client's current clinical status, nursing needs, and plan of care.
  • Communicate with families, nurses, physicians, case managers, and the agency clinical team regarding changes in the client's condition or care needs.
  • Review and ensure compliance with the client's Plan of Care and physician orders.
  • Provide clinical guidance and support to field nurses.
  • Identify changes in condition and communicate appropriate recommendations to the care team.
  • Ensure timely and accurate completion of supervisory documentation.
  • Maintain compliance with agency policies, Illinois regulations, and applicable pediatric home health requirements.
  • Travel to clients' homes throughout the assigned service area.

Qualifications

  • Current, unrestricted Illinois RN license — RN required.
  • Minimum 1 year of pediatric nursing experience required.
  • Home health experience preferred.
  • Experience with OASIS assessments preferred.
  • Experience with Axxess EMR is strongly preferred.
  • Strong clinical assessment and documentation skills.
  • Excellent communication and organizational skills.
  • Ability to work independently and manage multiple assigned clients.
  • Reliable transportation and valid driver's license.
  • Must be willing to commute up to 50 miles one way to assigned client homes.
