Registered Nurse (RN) – Start of Care and Home Health Visits | East Orlando, Orange County, FL

Matrix Home Care is seeking an experienced and dependable Registered Nurse to complete Start of Care assessments and skilled nursing visits throughout East Orlando and the surrounding Orange County area. Responsibilities include completing Start of Care assessments and initial patient evaluations, performing skilled nursing visits, reassessments, recertifications, resumptions of care, and discharge visits, completing comprehensive nursing assessments, developing and updating individualized plans of care, providing wound care, medication management, disease-management education, educating patients and caregivers, coordinating with physicians and clinical team, completing OASIS and electronic medical record documentation, and following agency policies and regulatory requirements.
