About this role
TRU Community Care is looking for a phone triage nurse for afterhours and triage team, taking calls from patients and collaborating with visit nurses on patient needs. Performs supportive role to Care Team by answering calls, monitoring and responding to triage calls, evaluating and assessing patients' healthcare needs using established triage protocols, obtaining provider input as needed, and directing patients to emergency room, urgent care, home care advice, or arranging in-home nursing visits.

Requirements
- Current RN license in Colorado or compact nursing license
- Minimum one year RN experience
- Hospice, home health, or PACE experience preferred
