First Call Urology (FCU) is a urology company delivering specialist-level urological care directly to patients living in skilled nursing facilities (SNFs) and long term care (LTC) communities —

without requiring residents to leave their beds.

The role

The Designated Urology Nurse is the clinical backbone of FCU's nursing home program. You will be the on-site urology specialist at a defined cluster of skilled nursing and long term care facilities in the

Northwest Indiana and Chicago metro area — the clinical presence that makes everything else possible.

You will work closely with FCU's urology providers, facility nursing staff, and Directors of Nursing to deliver a comprehensive urology care program that most nursing home residents have never had access to before. You will be trained on advanced urology-specific clinical skills and you will be a genuine clinical expert in your facilities, not just

a coordinator.

This role is field-based. You will travel between a defined cluster of facilities in the NWI/Chicago area daily.

What you will do

Clinical Care Delivery

• Conduct daily rounds across a defined cluster of skilled nursing and long term care facilities in the NWI/Chicago market

• Transport the FCU telehealth iPad from room to room, connecting residents directly to FCU urology providers for bedside video visits

• Perform Foley catheter insertions and changes per FCU protocol and physician orders

• Conduct bedside bladder scans (post-void residual measurements) and document results in real time

• Collect urine specimens via clean catch technique and straight catheterization when clean catch is not clinically feasible

• Administer antibiotic bladder instillations (bladder washes) for catheter-dependent residents with recurrent urinary tract infections per FCU standing orders

• Identify and escalate acute urology concerns to the FCU provider team promptly

Care Coordination & Chronic Disease Management

• Serve as the primary clinical liaison between FCU's urology providers and each facility's nursing team and administration

• Maintain accurate and timely clinical documentation
• Communicate with facility Directors of Nursing regarding patient status, care plan updates, and clinical concerns

• Attend monthly FCU case review and quality meetings

Staff Education & Training

• Educate facility nursing staff on proper Foley catheter care, maintenance, and infection prevention best practices

• Train nursing colleagues on early urinary tract infection symptom recognition and appropriate escalation pathways

• Serve as the on-site resource for urology-related clinical questions from facility staff

What we are looking for

Required

• Active, unrestricted Registered Nurse (RN) license in Indiana and/or Illinois — or the ability to obtain licensure in both states prior to start

• Minimum 2 years of clinical nursing experience
• Proficiency in Foley catheter insertion and management
• Proficiency in urine specimen collection techniques
• Comfort with tablet and mobile technology
• Valid driver's license and reliable transportation — this role requires daily travel between facilities in the NWI/Chicago area

• Strong communication skills — you will be the face of FCU at every facility you serve

• Clinical documentation discipline — accurate and timely
• Ability to work independently and manage your own daily schedule across multiple facility locations

Preferred (not required)

• Experience in skilled nursing, long term care, or post-acute care
• Urology nursing experience or exposure — inpatient, outpatient, or procedural

• Bladder scan experience (BladderScan or similar portable ultrasound device)

• Experience with chronic care management (CCM) documentation
• Wound care or IV certification (helpful but not required)
• BSN preferred — ADN candidates with strong LTC experience encouraged to apply
