What You’ll Do

  • Lead and coordinate individualized hospice care plans
  • Provide skilled nursing care including pain and symptom management
  • Supervise LVNs and CHHAs in the field
  • Collaborate with physicians and the interdisciplinary team
  • Educate and support patients and families during critical moments
  • Make a real impact every single day

Who You Are

  • Licensed Registered Nurse (RN) - BSN/MSN (preferred)
  • Experienced in hospice, home health, or case management (preferred)
  • Compassionate, independent, and confident in the field
  • Comfortable with EMR/documentation
