What You’ll Do
- Lead and coordinate individualized hospice care plans
- Provide skilled nursing care including pain and symptom management
- Supervise LVNs and CHHAs in the field
- Collaborate with physicians and the interdisciplinary team
- Educate and support patients and families during critical moments
- Make a real impact every single day
Who You Are
- Licensed Registered Nurse (RN) - BSN/MSN (preferred)
- Experienced in hospice, home health, or case management (preferred)
- Compassionate, independent, and confident in the field
- Comfortable with EMR/documentation
