About this role
Only Care Home Health is looking for a passionate and experienced Director of Nursing to lead our home health clinical team in Pasadena, CA. Responsibilities include overseeing all clinical operations and nursing staff, ensuring compliance with federal, state, and local home health regulations and Medicare/Medicaid standards, developing clinical policies and quality improvement initiatives, supervising and mentoring RNs, LVNs, and home health aides, collaborating with physicians and interdisciplinary care teams, monitoring patient outcomes and leading QAPI programs, managing staffing and scheduling, and serving as a liaison between clinical staff, administration, and external regulatory bodies.

Requirements
- Active RN license in the state of California
- Minimum 2-3 years of home health experience in a clinical supervisory or management role
- In-depth knowledge of Medicare/Medicaid home health Conditions of Participation (CoPs)
- Strong leadership, communication, and team-building skills
- Experience with OASIS documentation and home health EMR systems
- Bachelor's degree in Nursing (BSN) required; Master's degree preferred
- Proven track record in quality assurance, compliance, and clinical program development
- Valid California driver's license and reliable transportation
