Licensed Practical Nurse (LPN) - Home Health Nurse

*This is a PRN independent contractor opportunity. Patient assignments are offered based on patient needs, agency census, geographic location, required clinical competencies, and contractor availability. Hours and assignments are not guaranteed.*

Responsibilities

The LPN will provide skilled nursing services according to applicable physician/provider orders, the patient's established Plan of Care, agency policies and procedures, and the LPN's authorized scope of practice.

Responsibilities may include

• Provide skilled nursing visits in patients' homes under appropriate nursing supervision.
• Monitor and document vital signs, symptoms, and changes in patient condition.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Provide wound care and dressing changes according to applicable orders.
• Assist with diabetes management, including blood glucose monitoring and reinforcement of patient education.
• Provide education and reinforcement related to hypertension, diabetes, medication management, nutrition, safety, and chronic disease management.
• Provide G-tube/enteral feeding care and patient/caregiver education when applicable.
• Provide catheter care and other skilled nursing procedures within demonstrated competency and scope of practice.
• Provide IV-related nursing services when permitted by scope of practice, demonstrated competency, agency policy, and applicable orders.
• Reinforce patient and caregiver education established by the RN and/or healthcare provider.
• Observe for changes in the patient's physical, behavioral, or functional condition and promptly communicate concerns to the appropriate supervising nurse.
• Communicate professionally with patients, caregivers, physicians/providers, nursing leadership, and other members of the care team.
• Follow infection prevention practices, standard precautions, hand hygiene, nursing bag technique, and established safety procedures.
• Protect patient privacy and confidentiality and comply with HIPAA requirements.
• Promote patient dignity, independence, personal choice, safety, and participation in care.
• Complete all required clinical documentation accurately, thoroughly, and within established timeframes.

Documentation

*Clinical documentation will initially be completed using paper documentation while We’re No Different completes full integration with its electronic software platform.*

Contracted nurses must be comfortable completing clear, accurate, detailed, legible, and timely handwritten clinical documentation during this transition.

Agency-specific orientation and guidance regarding documentation requirements and forms will be provided. Once the electronic documentation system is fully implemented, contracted nurses will receive appropriate information and training for the transition to electronic documentation.

QualificationsRequired

• Current and unrestricted *Florida Licensed Practical Nurse (LPN) license* in good standing.
• Current *CPR/BLS certification*.
• Strong clinical observation, communication, and documentation skills.
• Ability to recognize and promptly report changes in patient condition.
• Knowledge of infection prevention, standard precautions, medication safety, and patient education.
• Ability to provide compassionate and professional care to patients with varying medical and support needs.
• Ability to independently manage accepted patient assignments while maintaining appropriate communication with agency nursing leadership.
• Ability and willingness to complete accurate paper clinical documentation during the transition to electronic documentation.
• Reliable transportation to assigned patient locations.
• Valid driver's license and current automobile insurance when driving to patient assignments.
• Ability to successfully complete required background screening, credentialing, orientation, training, and applicable clinical competency validation.
• Ability to provide and maintain required independent contractor credentials and documentation.

Preferred

• Previous home health, private duty, skilled nursing, rehabilitation, long-term care, or community-based nursing experience.
• Experience working with seniors and/or individuals with intellectual or developmental disabilities.
• Experience with diabetes, hypertension, medication management, wound care, G-tube/enteral care, catheter care, or other skilled nursing services.
• Experience providing patient and caregiver education.
• Strong organizational and time-management skills.

Contractors are responsible for maintaining all required professional credentials and documentation applicable to their assignments and for their own applicable taxes and other obligations associated with independent contractor status.
