Triage Registered Nurse
What You Must Have
- Current State Registered Nurse License
- Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance
- Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do
- Provide telephonic triage to hospice patients and their families during after- hours, weekends, or at other times when office is closed or at the request of the Executive Director
- Ensure after-hour visits are scheduled with the on-call team in a timely manner
- Ensure documentation is completed and submitted to the site at the end of each shift to ensure continuity of care for the week day team
- Manage referrals that come in over the weekend and after-hours
- Provide education and support to patients and families/caregivers
- Conduct patient visits as needed (admissions, routine, pronouncements, tuck-ins)
- Promote positive, respectful communication to all staff, referral sources, patients and families
- Performance of other duties as required
- Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
