Licensed Vocational Nurse - Home Health

Duties

  • Provide skilled nursing care to patients in their homes
  • Conduct health assessments and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Perform wound care and dressing changes

  • Assist with activities of daily living, such as bathing, grooming, and mobility
  • Educate patients and their families on managing their health conditions
  • Coordinate with other healthcare professionals to ensure continuity of care
  • Conduct health coaching and provide guidance on lifestyle modifications
  • Collaborate with interdisciplinary teams for discharge planning

Qualifications

  • Valid LVN license in the state of practice
  • Minimum of 1 year of nursing experience, preferably in home health or a related field
  • Knowledge of sonography, dermal fillers, behavioral health, ophthalmology is a plus
  • Familiarity with electronic medical record systems (eClinicalWorks, Athenahealth)
  • Ability to lift and move patients as needed
  • Proficiency in basic IV therapy techniques
