We are currently seeking a compassionate, hardworking, and committed Registered Nurse to join our Home Health team in either a full-time or PRN capacity.

As a Home Health RN, you will deliver skilled nursing care in patients’ homes—where comfort, dignity, and trust matter most. This role goes beyond clinical expertise; it’s about building relationships, educating patients and families, supporting independence, and making a real difference every day. Whether you are experienced in home health or new to the setting, we provide training to ensure your success.

Position Description | Registered Nurse (RN)

The Registered Nurse is responsible for coordinating and delivering comprehensive in-home nursing care. This includes direct patient care, patient and family education, and care coordination during transitions. The RN assesses patient needs, develops individualized plans of care and goals in accordance with physician orders, and oversees the planning, implementation, evaluation, and communication of all phases of nursing care for assigned patients.

The RN appropriately delegates care to other disciplines based on staff competencies, patient acuity, and the established plan of care. Participation in team meetings, case conferences, and in-service education is required to support collaboration and high-quality care delivery.

Qualifications

  • Minimum of one year of RN experience (home health experience preferred but not required)
  • Willingness to learn and provide a wide range of skilled nursing services in a home setting
  • Successful completion of background check

License or Certification Requirements

  • Current Colorado RN license
  • Current CPR certification
  • Valid driver’s license
