Immediate Home Health and Hospice is a leading provider of home health and hospice services in the Philadelphia, PA area. Seeking talented, passionate individuals to join the team and help patients live happier and healthier lives in their homes. Responsibilities include working collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to patients in their homes, conducting patient home visits, assessing and planning patient care plans, managing emergency situations, completing medication administration and treatment needs, providing nursing care in accordance with regulations and company policy, and maintaining accurate clinical and progress notes.
