Expectations
Responsible for supervising the daily operations and workflows of the Discharge Care Center (DCC) and transitional care services across the continuum of care. Oversees staff operations, workflow coordination, patient transition activities, and departmental processes that support continuity of care, patient engagement, readmission reduction, and post-discharge outcomes. Partners with interdisciplinary teams to support an integrated care coordination model spanning inpatient, ambulatory, post-acute, and community settings. Provides operational oversight, staff development, quality monitoring, and process improvement support in collaboration with the Executive Director of Case Management.

Education
Bachelor’s degree in nursing (BSN) from an accredited nursing program is required.
Experience/training
Minimum of three (3) years of clinical recent clinical experience in hospital case management, care coordination, transitional care, and discharge planning required.
Previous Lead, Charge Nurse, Preceptor, or supervisory experience preferred.
Licenses/
Certifications
Required
Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN) required.
Preferred
Case Management certification (ACM, CCM, or equivalent) preferred.
Skills
Knowledge of care coordination, transitional care management, discharge planning, and post-acute care workflows. Knowledge of regulatory and accreditation standards applicable to care management and transitions of care. Ability to supervise, coach, develop, and evaluate staff in a fast-paced healthcare environment.
Strong organizational, prioritization, and workflow coordination skills with the ability to manage multiple competing priorities. Ability to analyze operational, productivity, quality, and readmission data to support process improvement initiatives. Strong interpersonal and collaboration skills with the ability to work effectively across interdisciplinary teams and community partners.
Effective verbal and written communication skills, including documentation review and operational reporting. Knowledge of electronic medical records, care coordination platforms, and healthcare software applications.

Pay Range: The estimated base rate for this position is $137,490.00 - $209,679.00.
Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
