About this role
Lehigh Valley Companion Care Homes is seeking a compassionate and dedicated LPN ( Licensed Practical Nurse) to join our team on a PRN basis. The ideal candidate will deliver high-quality care in clients' homes and communities, ensuring their health and well-being are prioritized in a compassionate and comfortable environment.
Responsibilities
Provide skilled nursing care to patients in their homes and in the community setting. Administer medications and monitor treatment plans as prescribed. Perform health assessments, including vital signs, and document findings.
Assist with personal care tasks, as needed. Support patients with mobility and physical therapy exercises to enhance their overall well-being. Ensure patient safety and comfort through proper care and supervision.

Coordinate with family members and healthcare providers to ensure optimal care and support. Respond to medical emergencies as needed and provide first aid as required. Document all care provided and maintain accurate patient records to ensure comprehensive and up-to-date documentation.
Qualifications
Current and valid LPN license in the state of Pennsylvania. Minimum of 1 year of nursing experience in home care, long-term care, or similar healthcare settings preferred. Ability to work independently and manage patient care responsibilities. Compassionate and patient-centered approach to care. Strong communication skills and the ability to work well with clients, families, and other healthcare professionals. Ability to handle the physical demands of the job, including lifting and moving patients as needed Ability to work on an as-needed (PRN) schedule
