Safe Life Home Healthcare, LLC is seeking a dedicated and experienced Nurse Case Manager to join our team in Lombard, IL. This on-site position is essential in overseeing the quality of care provided by our field staff, ensuring coordination with physicians and clinicians to deliver the best possible outcomes for our patients. As a Home Health Nurse (Case Manager), your role will take you into the heart of our patients' homes.

You'll be responsible for delivering multifaceted care, ensuring they maintain their independence, and devising care plans tailored to each patient's unique diagnosis and requirements. Duties range from administering IVs at home, managing wounds, to keeping physicians updated about their patient's well-being. Responsibilities include comprehensive patient assessment, care plan curation, patient/caregiver education, medication management, specialized care (wound, IV, PICC Line, G-Tube, Foley care), and multidisciplinary team collaboration.
