About this role

Responsibilities
- Chart observations of the patient’s condition at each visit.
- Administer Physician-prescribed medication.
- Dress or redress wounds and monitor the healing progress.
- Provide education to patients and families on proper home health care procedures and strategies.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
- Recommend tools or devices that may improve the quality of life for the patient.
Qualifications
- Valid LPN license and in good standing
- Proven experience as a licensed practical nurse, preferably in home health
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to always follow them
- A team player with excellent communication and interpersonal skills
- Responsible and compassionate
- Outstanding organizational and multi-tasking skills
- Patient with excellent problem-solving skills.
