About this role

*We are seeking an experienced Wound Care Nurse ( RN / LPN ) to join our Parkwood Skilled Nursing team Full Time. As part of our nursing team, you will provide wound care to residents in long-term and rehabilitiation environments. Our Wound Care Nurse is responsible in creating and maintaining a plan that results in successful wound care outcomes. *

  • Must have prior experience in assessing and caring for wounds
  • Assisted Living, Long Term Care/SNF or related experience a plus
  • Experience with Alzheimer’s and dementia is a plus
  • Be a self-starter and able to work independently
  • Ability to establish effective relationships with residents, family members and staff

*Wound Care Nurse - RN / LPN

Responsibilities

*

  • Proactively assess, treat and chart on wound care service and progress
  • Work on the floor as needed
  • Chart on all residents under your care.
  • Take on-call rotation with other nurse managers.
  • Responsible for nursing care of all residents during his/her shift.
  • Supervision of the nursing department and performance of staff assigned to his/her zone or hall including nursing assistants and medication aid.
  • Provide education and guidance to the staff under your assigned area.
  • Take verbal and written orders from medical personnel and transcribe, note and follow up on the orders and document the nursing notes.
  • Continue a professional relationship with the medical professionals of the facility.
  • Performs other duties as assigned.
