About this role
The Palliative Care Registered Nurse (RN) supports VITAS Healthcare's growing palliative care program within the assigned service area. The selected candidate coordinates care for assigned patients, completes physical and psychosocial assessments, ensures appropriate referrals, assists patients in accessing community services, educates patients and families on disease processes and care plans, provides emotional support and counseling, evaluates patient recovery, empowers patients in advance care planning, partners with healthcare team for transitions of care, and facilitates discharge planning.

Requirements
- RN
- Driver's License
- Minimum 2 years' experience as Registered Nurse
- 1-2 years of nursing experience in a community-based setting preferred (home health, rehabilitation, hospice, or palliative care)
- Bachelor's degree preferred
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
- Reliable transportation, current state driver's license and automobile insurance
- Bilingual a plus
- Exceptional communication and customer service skills
