*Position Summary*

We’re seeking a *Registered Nurse (RN)* to join our clinical team. The RN is responsible for the delivery and coordination of patient care services, ensuring that the Plan of Care is implemented effectively and in compliance with state and federal regulations. This position offers flexible scheduling and the opportunity to make a meaningful difference in patients’ lives in the comfort of their own homes.

*Responsibilities*

• Perform initial evaluations, recertifications, resumptions of care, and discharges.
• Develop and implement individualized Plans of Care in collaboration with physicians and interdisciplinary team members.
• Supervise LVNs, HHAs, and other staff assigned to the patient’s care.
• Conduct in-home supervisory visits at least every 14 days for patients receiving aide services.
• Monitor and update patient medication records and treatment documentation.
