Responsibilities

  • Develop, implement, and evaluate case management plans for patient populations, coordinating care across disciplines.
  • Advocate for patients and families; communicate and collaborate with physicians, nurses, social workers, and payers to optimize care.
  • Document Case Management plans clearly, including all patient contacts and care decisions.
  • Facilitate appropriate utilization and discharge planning; participate in interdepartmental meetings related to Case Management/Social Services.
  • Collaborate with payers and review organizations to ensure proper resource use and adherence to regulatory requirements.
  • Maintain current knowledge of discharge planning, utilization processes, and case management best practices through ongoing education.

Requirements

  • Current, unrestricted Registered Nurse license in Colorado
  • Bachelor’s Degree in Nursing; Master’s degree preferred
  • At least three (3) years of clinical experience in a hospital setting and at least five (5) years of case management experience in a hospital setting
  • Case management certification preferred or willingness to obtain within two (2) years of employment
  • Ability to document and communicate clearly; strong collaboration with physicians, patients, families, and staff
  • Willingness to fulfill weekend coverage and participate in the described weekend rotation
