Wise Medical Staffing is seeking an experienced Registered Nurse (RN) Case Manager for a full-time Home Health opportunity in Baltimore, MD. This position is responsible for coordinating and managing the care of patients across the lifespan in the home setting. The RN Case Manager serves as the primary clinical coordinator, ensuring high-quality patient care through comprehensive assessments, individualized care planning, interdisciplinary collaboration, and ongoing patient education. The ideal candidate is organized, compassionate, clinically confident, and experienced in Home Health nursing and case management.

Requirements

Active and unrestricted Maryland Registered Nurse (RN) license required
Minimum 2 years of Home Health nursing experience preferred
Previous Case Management experience preferred
OASIS experience required or preferred (depending on client requirements)
Current BLS/CPR certification required
Valid driver's license, reliable transportation, and current auto insurance
Ability to travel throughout the Baltimore service area
Strong assessment, critical thinking, and care coordination skills
