Requirements
- Active and unrestricted Maryland Registered Nurse (RN) license required
- Minimum 2 years of Home Health nursing experience preferred
- Previous Case Management experience preferred
- OASIS experience required or preferred (depending on client requirements)
- Current BLS/CPR certification required
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the Baltimore service area
- Strong assessment, critical thinking, and care coordination skills
- Excellent communication and documentation skills
- Ability to pass background screening and onboarding requirements
Responsibilities
- Complete comprehensive nursing assessments for patients of all ages in the home setting
- Develop, implement, and update individualized plans of care
- Coordinate patient services with physicians, therapists, caregivers, and interdisciplinary healthcare teams
- Complete OASIS assessments and maintain accurate clinical documentation
- Monitor patient progress and adjust care plans as appropriate
- Educate patients and caregivers regarding diagnoses, medications, treatments, and disease management
- Ensure compliance with Medicare Conditions of Participation and agency policies
- Participate in case conferences and quality improvement initiatives
- Maintain timely documentation and communication with referral sources and physicians
- Deliver compassionate, patient-centered care while promoting independence and positive health outcomes

Wise Medical Staffing is seeking an experienced Registered Nurse (RN) Case Manager for a full-time Home Health opportunity in Baltimore, MD.
This position is responsible for coordinating and managing the care of patients across the lifespan in the home setting. The RN Case Manager serves as the primary clinical coordinator, ensuring high-quality patient care through comprehensive assessments, individualized care planning, interdisciplinary collaboration, and ongoing patient education.
The ideal candidate is organized, compassionate, clinically confident, and experienced in Home Health nursing and case management.
