Position Summary

As a Hospice Registered Nurse, you will provide skilled nursing care to patients in their homes while coordinating care with an interdisciplinary team to ensure patients receive the highest quality end-of-life care. This position offers the opportunity to build meaningful relationships with patients and families while making a lasting impact in your community.

Essential Responsibilities

• Provide skilled nursing care in accordance with physician orders, hospice regulations, and agency policies.

• Complete comprehensive patient assessments and develop individualized plans of care.

• Coordinate patient admissions, recertifications, transfers, and discharges.

• Educate patients and families regarding disease progression, symptom management, medications, and hospice services.

• Collaborate with physicians, Nurse Practitioners, Medical Directors, Social Workers, Chaplains, Home Health Aides, and other members of the interdisciplinary team.

• Supervise and provide clinical direction to LVNs, Home Health Aides, and other assigned staff.

• Participate in weekly Interdisciplinary Team (IDT) meetings.

• Complete accurate and timely electronic documentation.

• Monitor changes in patient condition and communicate appropriately with the care team.

• Advocate for patients and families while promoting dignity, comfort, and quality of life.

• Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.

Qualifications

• Current, unrestricted Texas Registered Nurse (RN) license.

• Graduate of an accredited Diploma, Associate Degree, or Bachelor of Science in Nursing program.

• Minimum one (1) year of Hospice or Home Health experience preferred.

• Current CPR certification.

• Excellent assessment, critical thinking, and clinical decision-making skills.

• Strong communication and interpersonal skills.

• Ability to work independently while collaborating effectively with an interdisciplinary team.

• Proficient in electronic medical records and computerized documentation.

• Valid Texas driver's license, reliable transportation, and proof of automobile insurance.

Schedule

• Full-Time or PRN

• Monday through Friday

• Day Shift

• Participation in an on-call rotation, including evenings, weekends, and holidays, as needed to ensure continuity of patient care.
