Responsibilities

Perform comprehensive patient assessments (initial, resumption, and ongoing visits)Develop, implement, and update individualized care plansAdminister medications, treatments, and wound care as orderedMonitor patient progress and report changes to physicians and care teamProvide patient and caregiver education on disease management and care plansComplete accurate and timely documentation in EMR (e.g., Kinnser EMR)Ensure compliance with Medicare/Medicaid guidelines and agency policiesCoordinate care with therapists, aides, and other healthcare professionalsParticipate in case conferences and quality improvement initiatives

Requirements

Active Florida Registered Nurse (RN) licenseMinimum 1 year of nursing experience (home health experience preferred)Knowledge of Medicare home health regulations and documentation standardsStrong clinical assessment and critical thinking skillsExcellent communication and interpersonal skillsAbility to work independently and manage a flexible scheduleReliable transportation, valid driver’s license, and auto insuranceCurrent CPR certificationPreferred

Experience with OASIS documentationFamiliarity with home health EMR systems (e.g., Kinnser EMR)Pay: $55.00 - $60.00 per hour
