A well-established and growing home health organization is seeking an experienced *Director of Patient Care Services* to provide clinical leadership and operational oversight of home health services.

This role is responsible for ensuring the delivery of safe, high-quality, and compliant patient care while supporting clinical teams through effective supervision, mentorship, and performance management. The Director will partner closely with operations and executive leadership to ensure regulatory readiness, strong clinical outcomes, and an exceptional patient and employee experience.
Key Responsibilities
- Provide clinical leadership and oversight of daily home health operations in accordance with professional standards and state and federal regulations
- Oversee coordination of patient care including referrals, admissions, assessments, care planning, transfers, and discharges
- Ensure development, implementation, and evaluation of individualized plans of care
- Review and approve clinical documentation, including OASIS assessments as required
- Supervise, coach, and mentor clinical leadership and field staff
- Ensure staff meet licensure, training, competency, and compliance requirements
- Lead quality and performance improvement initiatives (QAPI), audit readiness, and regulatory compliance efforts
- Monitor clinical performance metrics, patient outcomes, and satisfaction data, implementing improvement strategies as needed
- Partner with operations leadership on staffing models, productivity standards, and resource allocation
- Provide leadership during emergency preparedness planning and response
- Serve as escalation point for complex clinical and operational matters
Qualifications
- Current Registered Nurse (RN) license in the practicing state
- Current CPR certification
- Minimum of two (2) years of home health experience required
- Must have Director Patient Care Services or Clinical Supervisor experience.
- Home health 3+ years in management
- Minimum of two (2) years of clinical leadership, management, or supervisory experience preferred
- Strong knowledge of home health regulations, OASIS, and compliance standards
