This position will be located and serve the Chelmsford, MA area. Clinical Team Manager provides oversight of all patient care services and personnel including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.

Requirements
- RN
- Associate degree in a Nursing related field required
- 3-5 years of Skilled Home Health Experience including field visits and case management
- Experience with Oasis documentation and assessments
- 1 year of supervisory experience preferred
- Current, unrestricted RN license valid for the state of work
