Department Summary

UNT Health Clinical Practice Group’s Center for Older Adults provides comprehensive care plans that help prevent and manage the chronic diseases and long-term illnesses that may accompany the aging process. Services include:

Comprehensive Geriatric Assessment - Cognitive and neuropsychological testing in Fort Worth - Primary care - Alzheimer’s and Dementia evaluation and treatment - Well care for older adults - Onsite care at several of Fort Worth’s continuing care retirement communities - Palliative care - Chronic care management (CCM services) - Neurology consultations for brain health evaluation, memory loss, dementia and movement disorders - Advance Care Planning.

Position Overview

HSC Health is seeking a compassionate LVN to join our dynamic team of healthcare providers at the University of North Texas Clinical Practice Group with the Center for Older Adults. This role requires a candidate with strong organizational skills, a wide set of interests and skill set, and strong interpersonal communication abilities. Candidate must have the ability to assess patients quickly and accurately. Exhibits compassion, understanding, the ability to work well in teams, and the ability to function in a fast-paced environment is critical for this position.

Minimum Qualifications

Graduation from an accredited school of nursing and one (1) year of related experience; or any equivalent combination of education and experience. A valid Texas Licensed Vocational Nurse (LVN) license and a current CPR (BLS) certification are required.

Knowledge, Skills and Abilities

A licensed vocational nurse (LVN) provides medical care to patients under the supervision of providers and registered nurses. Knowledge of applicable safety requirements. Knowledge of basic first aid and CPR certification.

Ability to react quickly and calmly in emergency situations. Ability to work successfully as a member of a team and independently with moderate supervision. Ability to communicate effectively in both oral and written form.

Ability to diffuse and respond effectively to situations involving intense pressures and/or unpredictable persons. Ability to handle difficult and stressful situations with professional composure. Ability to effectively handle difficult and/or angry people.

Skill in talking to others to convey information effectively. Ability to process and handle confidential information with discretion. Skill in completing assignments accurately and with attention to detail.

Preferred Qualifications

The preferred candidate will possess the following additional

qualifications

Bilingual competency (English/Spanish). Previous experience preferably with a geriatric population including telephone communication, ambulatory care, familiarity with adult vaccine schedule, patient rights, and safety. Experience with Medicare and Managed Medicare insurance plans.

Required License/Registration/Certifications

The successful candidate will possess the following required

qualifications

Graduation from an accredited school of nursing. A valid Texas nursing License and a current CPR (BLS) certificate are required.

Job Duties

Measure and record patients' vital signs, perform procedures, and administer medications, injections, and vaccines as ordered by the clinician in the electronic health record. Assembles, maintains, and properly uses equipment in patient care delivery. Return patient calls, and performs telephone triage following standardized protocols and clinician's guidance.

Gauges patients' overall health status, updating clinicians as indicated. Provides oversight and guidance to medical assistants in collaboration with clinic supervisor/manager. Obtains insurance authorizations for referrals, procedures, and medications.

Follows clinical protocols for medication refills. Completes patient forms as necessary. Follows all departmental and organizational policies and procedures with regard to patient safety, quality management, infection control, and human resources.

Ensures appropriate legal and regulatory standards. Deliver culturally competent care that respects each patient's individual beliefs. Participate in process and quality improvement initiatives that aim to ensure patient safety and efficient clinical workflows.

Coordination of care with internal and external partners.

Physical Requirements

Repeating motions that may include the wrists, hands and/or fingers. Sedentary work that primarily involves sitting/standing. Lifting and Moving objects up to 20 pounds. Communicating with others to exchange information.
