Home Health Registered Nurse (RN). Serving Dorchester, Charleston, and Berkeley Counties. Interim Healthcare is seeking a dynamic home health registered nurse to join their team. Responsibilities include ongoing interdisciplinary assessment of patients, initial and ongoing comprehensive assessments, development and evaluation of plan of care, providing physician-ordered services, counseling and educating patients and families, initiating preventative and rehabilitative nursing procedures, monitoring outcomes, supervising paraprofessional staff, and participating in QAPI program.

- Clinical proficiency in nursing, strong communication skills, and compassion
