About this role
Hospital Care at Home is an innovative care model that allows us to provide inpatient level care to patients in their homes. The Home Hospital RN will partner closely with the attending physician and other members of the care team to provide care for patients, both virtually and through in-home visits. Primary responsibilities include participating in initial and ongoing assessment of patients, collaborating with medical providers in planning and implementation of patient care plan, directing and coordinating services provided in the home, collaborating with clinical team for remote monitoring services, acting as central point of contact for all patients, developing and coordinating individual nursing plan-of-care including patient/family education and discharge planning, driving to patients' homes in a university-provided vehicle to provide in-home care, and participating in ongoing improvement initiatives.

Requirements
- RN
- Driver's License
- Degree in Nursing from CCNE or ACEN accredited nursing program
- Current Ohio RN license or interim permit
- Minimum of 3 years clinical experience in acute care setting
- Valid United States or Canadian driver's license
