We are seeking a compassionate and experienced Per Diem Registered Nurse to join our dedicated team at A Place At Home Somerville. As a Per Diem RN, you will provide high-quality, patient-centered care to our clients in the comfort of their own homes. About this Role

Conduct initial and ongoing assessments to identify client needs and develop effective care plans.

Collaborate with clients, families, and caregivers to ensure seamless transitions and optimal outcomes.

Conduct caregiver supervisory visits in compliance with state regulations and agency policies.

Assess caregiver competency and provide coaching, education, and support as needed.

Utilize strong communication and interpersonal skills to manage client expectations and maintain positive relationships

Requirements

Active RN license in the state of NJ

Bachelor’s degree in Nursing (preferred)

Minimum 2 years of clinical experience, ideally in home care

Reliable transportation and auto insurance

Clean background and criminal record

CPR/BLS certification
