Patients remain formally admitted to a hospital partner while receiving coordinated in-home care supported by physician oversight and continuous clinical monitoring.

Position Summary
Provide hospital-level skilled nursing care to patients enrolled in a Hospital at Home program. RNs deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky).
Key Responsibilities
Deliver hospital-level clinical care in the patient’s home
Complete timely, accurate, and thorough clinical documentation in Kinnser (WellSky) NO OASIS REQUIRED
Communicate patient status changes per established clinical protocols
Adhere to program workflows, response expectations, and professional standards
Provide care that reflects hospital-level quality and accountability
Required Qualifications
Active, unrestricted professional license
Strong assessment, clinical judgment, and documentation skills
Ability to work independently while meeting hospital-level expectations
Comfort with mobile technology and electronic documentation systems
Reliable transportation and strong attendance record
Preferred Experience
Acute care, hospital, ICU, ER, or step-down experience
High-acuity home health or complex medical care experience
Experience collaborating with interdisciplinary clinical teams
