Responsibilities

• Plans and implements patient health teaching and health counseling regarding disease process(es), self-care techniques, and prevention, involving the patient's significant others in accomplishing health goals while promoting patient/family independence

• Performs patient admission assessment, recertification, subsequent visits, and discharge visits

• Completes and reviews Health Risk Screenings (HRST) and develops Individual Nursing/Health Care Plans for individuals with ongoing health needs

• Delegates and oversees nursing tasks performed by trained direct-support staff, consistent with the Georgia RN/LPN Nurse Practice Act

• Oversees medication administration, reviewing Medication Administration Records (MARs) for accuracy and following up on any concerns

• Reports all visits and changes in patient status to the RN Case Manager assigned to the patient

• Coordinates with physicians and support coordinators on orders, appointments, and follow-up care

• Manages and coordinates all aspects of service delivery as defined by physician orders

• Strives to provide services that meet both internal and external customer expectations

• Assumes individual responsibility for professional competence and professional nursing practice

Qualifications

• Valid RN license (Georgia)

• Proficient in patient care and assessment

• Ability to provide basic life support and health coaching

• No prior experience with Health Risk Screening Tools (HRST) or waiver/home-health programs required — we provide full training and competency verification on each before you take it on, with annual refreshers to keep you current

Job Type: Part-time Pay: $30.00 - $51.00 per hour
