What You Will Do
Patient-Centered Care Coordination: Implement and monitor individualized care plans tailored to unique clinical needs, ensuring all interventions align with the patient's personal goals and recovery strategy.
Interdisciplinary Collaboration & Advocacy: Serve as the primary liaison between patients, families, and the care team to facilitate seamless communication and advocate for high-quality, aligned care.
Clinical Monitoring & Execution: Provide high-standard clinical support by accurately assessing health status, documenting changes, and executing physician orders with precision to support the overall treatment strategy.
Continuity of Care & Discharge Planning: Lead comprehensive discharge and transition efforts, empowering patients and their support networks to move confidently toward the next stage of healing.
What You Will Need
Licensure / Certification / Registration
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
Education
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
