About the Role

Clarvida is seeking a Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team. The RN will work closely with adults who experience serious and persistent mental illness to provide individualized care that addresses the client’s medical needs, mental health support, and overall well-being. In this role, you will conduct nursing assessments, monitor health status, assist with medication administration, and support clients in managing their physical and mental health needs.

You will work primarily in the community, meeting with clients in their homes, in the community, and in outpatient settings. You will be part of a multidisciplinary team, collaborating with other health professionals to ensure the best possible care for each individual.

## Responsibilities

  • Conduct nursing assessments and health histories
  • Monitor clients’ responses to medications and health conditions
  • Assist with medication management and applying for Patient Assistance Program medications
  • Educate clients on self-administering medications
  • Work with clients and their families to address medical, nutritional, and other health issues
  • Provide on-site care, and collaborate with the ACT team to ensure the best treatment outcomes
  • Support clients with physical health and substance use challenges in addition to their mental health needs

## Requirements

  • Licensed as a Registered Nurse (RN) in Georgia
  • 1+ year of experience in a psychiatric setting (preferred)
  • Ability to pass all background checks, including Enhanced Fingerprinting with ID DHW
  • Valid Georgia Driver’s License and clean Motor Vehicle Record (MVR)
  • Clean criminal background check (no conviction or credible evidence of abuse, neglect, or sexual exploitation of children or adults)
