Registered Nurse (RN) – Adult Day CareOverview
Join our healthcare team as a *Registered Nurse (RN) in an Adult Day Care setting*, where you will play an important role in supporting the health, safety, and well-being of our participants. The RN will provide nursing assessments, medication management, health monitoring, care coordination, and participant education while working closely with the interdisciplinary team.
This position is ideal for a compassionate, organized nurse who enjoys working with older adults and individuals who may have chronic medical conditions or developmental disabilities.
*Spanish-speaking ability is a plus.*

Responsibilities
- Perform nursing assessments and monitor participants’ overall health status.
- Check and document vital signs, blood glucose levels, weight, and other health indicators as needed.
- Administer medications and treatments according to physician orders and facility policies.
- Review medications and monitor participants for side effects, changes in condition, or potential health concerns.
- Maintain accurate and timely nursing documentation and participant medical records.
- Communicate changes in participant condition to physicians, families, caregivers, and appropriate members of the interdisciplinary team.
- Develop, review, and update individualized nursing care plans.
- Provide health education to participants and caregivers regarding medications, chronic disease management, nutrition, safety, and preventive care.
- Respond appropriately to medical emergencies and provide first aid or emergency nursing care when necessary.
- Assist with infection prevention and control practices.
- Coordinate with the Program Director, Administrator, social services, dietary staff, activity staff, and other members of the care team.
- Participate in care plan meetings and quality improvement activities.
- Ensure compliance with applicable Adult Day Health Care policies, procedures, and regulatory requirements.
Requirements
- Current and valid *California Registered Nurse (RN) license* in good standing.
- Current CPR/BLS certification.
- Experience working with older adults, individuals with chronic medical conditions, or participants with developmental disabilities preferred.
- Adult Day Health Care, skilled nursing, home health, rehabilitation, or community health experience is preferred.
- Strong assessment, medication management, documentation, and communication skills.
- Ability to recognize changes in participant condition and respond appropriately.
- Ability to work collaboratively with an interdisciplinary healthcare team.
- Compassionate, dependable, organized, and able to work independently when needed.
- Basic computer and electronic medical record/documentation skills.
- *Bilingual English/Spanish is a plus.*
