Registered Nurse

Responsibilities

• Perform nursing assessments and monitor participants’ overall health status.
• Check and document vital signs, blood glucose levels, weight, and other health indicators as needed.
• Administer medications and treatments according to physician orders and facility policies.
• Review medications and monitor participants for side effects, changes in condition, or potential health concerns.
• Maintain accurate and timely nursing documentation and participant medical records.
• Communicate changes in participant condition to physicians, families, caregivers, and appropriate members of the interdisciplinary team.
• Develop, review, and update individualized nursing care plans.
• Provide health education to participants and caregivers regarding medications, chronic disease management, nutrition, safety, and preventive care.
• Respond appropriately to medical emergencies and provide first aid or emergency nursing care when necessary.
• Assist with infection prevention and control practices.
• Coordinate with the Program Director, Administrator, social services, dietary staff, activity staff, and other members of the care team.
• Participate in care plan meetings and quality improvement activities.
• Ensure compliance with applicable Adult Day Health Care policies, procedures, and regulatory requirements.

Requirements

• Current and valid *California Registered Nurse (RN) license* in good standing.
• Current CPR/BLS certification.
• Experience working with older adults, individuals with chronic medical conditions, or participants with developmental disabilities preferred.
• Adult Day Health Care, skilled nursing, home health, rehabilitation, or community health experience is preferred.
• Strong assessment, medication management, documentation, and communication skills.
• Ability to recognize changes in participant condition and respond appropriately.
• Ability to work collaboratively with an interdisciplinary healthcare team.
• Compassionate, dependable, organized, and able to work independently when needed.
• Basic computer and electronic medical record/documentation skills.
• *Bilingual English/Spanish is a plus.*
