We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN at Kings Harbor Multicare Center, you will be responsible for providing personalized care to our residents, including administering medications, performing vital signs, and assisting with daily living activities. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
Responsibilities
Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct resident assessments and evaluations to identify care needs and develop strategies for meeting those needs
Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care coordination
Maintain accurate and up-to-date records of resident care, including medication administration and treatment plans
