Daily responsibilities include coordinating and managing cases, participating in outreach clinics, following up with individuals and families to ensure continuity of care, and documenting services in accordance with department policies and public health regulations. The Public Health Nurse collaborates with interdisciplinary teams, community partners, and residents to address public health concerns, respond to outbreaks, and support vulnerable populations. The position also involves contributing to program planning, quality improvement initiatives, and public health campaigns.

Qualifications Candidates should possess strong skills in Public Health Nursing and Community Health, with the ability to deliver population-focused interventions and community-based services. Candidates should possess Nursing and Medicine skills, including conducting assessments, providing evidence-based care, and following clinical guidelines and protocols. Candidates should possess Case Management skills, including coordinating services, monitoring progress, and advocating for individuals and families across systems of care.

Active Registered Nurse (RN) license in the state of Wisconsin, or eligibility for licensure, is required. Experience in community or public health settings, such as local health departments, clinics, or outreach programs, is highly beneficial. Strong communication, cultural humility, and interpersonal skills, with the ability to work effectively with diverse populations and interdisciplinary teams.

Proficiency in documentation, basic computer skills, and use of electronic health records or public health information systems. Bachelor’s degree in Nursing (BSN) preferred; additional training or certification in public health or community health nursing is an asset.
