Job Overview
We are seeking a dynamic and energetic Bilingual Team Lead fluent in Russian and English to provide continuous support and communication between healthcare professionals, patients, and their families. Under the direction of a Licensed Clinician, the role is responsible to schedule or coordinate scheduling of necessary visits, tests, and/or treatments, gather patient information, execute individualized care plans, build a rapport with patients as their advocate and provide ongoing support to both patients and their caregivers. - Initiates welcome calls to introduce the HNYMPC’s Program, build rapport, and inform patient/caregiver of the benefits of care management to provide a positive patient experience
- Actively communicates upon as required to explain coverage benefits, obtains accurate patient/caregiver contact information and continually update and maintain accurate patient demographics
- Telephonically complete questionnaires to gather essential information which allows the Clinician to refer patients to medical, mental health, social workers and/or pharmacists in a timely manner
- Assists patients/caregivers in finding providers in their geographic areas, as well as scheduling appointments for the patients
- Available by phone to assist the patients/caregivers in calling provider’s offices, faxing forms, requesting documentation.
- Participate in meetings as assigned (Regional, ICT, etc.)
- Execution plan of care by assisting patient/caregiver in following up with prescribing provider and DME vendor to ensure that necessary DME is received, and the patient/caregiver feels comfortable using it
- Sends out basic health education materials as well as advanced directives and health care proxy forms
- Available to address issues, answer questions, provide resource, telephone numbers and/or to transfer patient/caregiver to appropriate staff
- Telephonically complete to gather essential information which allows the Care Manager to refer patients to medical, mental health, social services, social workers and/or pharmacists in a timely manner.
- Actively assist the Clinicians by gathering specific information which helps track the patients progress on a regular basis towards the mutually agreed upon goals in the patients care plan
- Actively assist the Care Manager in transition of care by gathering pertinent information when initiating the Hospital and SNF Discharge calls
- Call PCP office to notify PCP of transition of care events as assigned
- Actively assist with referrals, by calling PCP/provider to ensure that patient is able to receive the necessary services in a timely manner
- Assist patients/caregivers in finding providers in their geographic areas, as well as scheduling appointments for the patients
- Assists with coordinating transportation for the patient
- Follow up with patient after scheduled appointment
- Maintain strict confidentiality of patient information and adhere to all HIPAA regulations
- Other duties, as assigned
*Qualifications*
- High School Diploma, required
- Bachelor’s degree preferred
- Fluency in Russian and English is REQUIRED with excellent communication skills in both languages.
- Excellent written and oral communication skills, knowledge of medical terminology.
- 6 months – 1 yr. experience working telephonically with outbound & inbound calls; Health Plan / Physician Office – All line of business population preferred.
- Must be able to prioritize, plan, and handle multiple tasks / demands simultaneously.
- Reliable Interdisciplinary Team Player, Detail Oriented and Flexible.
- Knowledge of Health Plan benefits/guidelines a plus.
- Proficient with Microsoft Office applications, Word, and Excel.
Pay: $50,000.00 - $75,000.00 per year
