About this role
Performance Home Health (PHH) is seeking experienced and motivated Part-Time Registered Nurses (RN) to join our home health team. The Home Health RN is responsible for providing skilled nursing care to patients in their homes while meeting PHH clinical, documentation, and productivity expectations. This role requires strong clinical skills, accountability, and the ability to work independently while maintaining consistent communication with clinical leadership and the interdisciplinary team.
Key responsibilities include performing comprehensive patient assessments, developing individualized plans of care, and providing skilled nursing services such as wound care, wound vac management, port care, injections, catheter care, IV therapy, and picc line care.
